RPM issue, will not go above 4k on tach

On a stock fzr 600, the tachometer will not go above 4k. The bike will rev higher, but the tach will not display the correct RPM's. not sure if this is affecting other performance on the bike since it is showing less than 13.7v at the battery when running. I've tried 2 VR's with the same results.

Unplug the tach, then measure the voltage across the battery. If the voltage goes to something reasonable (14.3-15.3 @ 5000 rpm), then the tach is well and truly pooched.
