We had a welder here at my work years ago who was just closing his evey when doing a tack weld with a mig welder. He forgot to close his eyes one time and wound up having to have his corneas scraped.
Needless to say he used a welding hood from there on out.

Yep, and teach this un-humble servant that when the old timers tell you to lose the auto-shade helmet and get a fixed shade, a person should listen. The guy's site is pretty good. Lots of 'field tested' stuff. I like how he hammers home that you need to learn uphand MIG for anything decently thick. A man can/will get fired at my shop for running downhand on anything over 11 guage.
Here's some photos of my latest D1.1 structural steel certification coupons. I have been laid off long enough (more than 6 months) that my certs needed to be updated. I ran coupons in 3g (vertical uphand GMAW{MIG}) and 4g (overhead-upside down-GMAW). The process itself is certified by our shops WPS (weld procedure specifications-includes voltage, wire, wire speed, gas and material)
This is a 3/8" steel(a36) groove weld, done with .045" e70-s6 flux cored wire. The groove is 1/4" wide, 37.5° bevel(with backer bar). It is a root weld, and then a 'fill pass' and then a cover pass. The cover cannot exceed 5/8" wide and 1/8" high. It was 7" long. The face pass and the root pass are sawn off from below and above the visual inspection section, sanded flat and then bent in a press(one to stretch the face, one to stretch the root) to expose any flaws. You can see in my 3g root there are a couple of inclusions, but they are under the total allowance by 3/32" (none can be OVER 1/32" and there is a total factor as well). Inclusions are allowed on root, but not internal, or face pass.
